- The type of membership you are looking for (Adult / Youth / full-time Student). Please note we do not offer any youth/junior programs, and youth/junior members must join with an adult member.
- Your home address
- Your approximate tennis skill level (if you’re unsure, this document can help you determine your approximate level)
